At least it's the 4th weekend of the month, so 3 days gone out of the whole month, instead of every single Wednesday night (blurg) or every other weekend or something.  Unless the "every 4th weekend" will eventually turn into "every other weekend", which would be super shitty.

Ugh, I've had to be on call but not really have to be glued to my email. Generally it was a "we'll call you" if anything went down. I guess technically it hasn't ended and I'm still part of a team that deals with releases but I've been called, like, once and that was over a year and a half ago at this point (probably longer, I don't remember).

That sucks though, especially with it being dropped on you for something you haven't done in forever. I call bullshit on that and the not being given a work phone. Every 4th weekend is probably the option I would've taken as well. Easier to deal with it once in awhile rather than it being a constant weekly annoyance. Here's hoping to not having to called upon much!

Yeah I guess that's worth mentioning.  My job has always required that if something really important goes wrong over a weekend I am expected to answer my phone and possibly work.  The difference is that folks have known that I'm not on work e-mail over the weekend so if it's that important, just that, they have to call me.

It's that this places onus on me to be aware of the e-mail and how disruptive that can be to a weekend or what we want to do in it.  I mean most of my weekends I'm not far from home so whatever, but if that happened to be the weekend we decided to, I dunno go on a long bike trip or bus down to Maryland stadium for a football game, neither are those quick "oh shoot let me run home" or "oh let me bring my laptop with me situations."

Oh man, work was rough yesterday. And I ended up missing an IT department dinner at one of the best burger places in Chicago because of it. Not happy about that (but at least I was able to attend Game Night). And once again I apologize for all the jargon I'm about to use.

So the short of it is that Monday at like 1something in the morning the password for the Primary Domain Controller's computer account was changed and something went wrong (I could see the pwdLastSet attribute had that timestamp). This broke Secure Channel, which is what the domain controllers (and other things that query them for domain-ish stuff) need to talk to them. As time went by, various already issued authentication tokens expired and were unable to renew, thus the slowly worsening trickle of symptoms we saw (WMI alerts in monitoring on that server, Radius authentication having trouble [though I don't know why this was intermittent instead of constant], inability for the various AD management pieces of software to connect with the PDC, inability to set folder and file permissions on the file server using domain accounts and groups, etc.)

Fixing schannel is done by simply resetting the password on the computer account, but it was complicated by the fact that it was the Primary Domain Controller that broke. On a secondary domain controller you do it one way (disable KDC service on broken DC and reboot, sign into PDC and run "netdom resetpwd /server:server_name /userd:domain_name\administrator /passwordd:administrator_password", re-enable KDC service on broken DC and reboot again), but that doesn't work for the PDC, and I had to find that out the hard way. I spent like 2.5 hours attempting it in various fashions (bringing down internal DNS each time) without success before accidentially finding out the other way. On the PDC you have to run the "netdom resetpwd /server:server_name /userd:domain_name\administrator /passwordd:administrator_password" command, then restart the Active Directory Domain Services service (it will have several dependencies that will also restart).

I missed out on a dinner and drinks that was paid for by work, as well as lost like an hour and a half of my evening last night to this. But at least it is working again now.

I'm supposed to be in a Red Hat System Administration class today, but somehow the training company I purchased it through and the people actually doing the training have had a miscommunication. I've been asked to leave the classroom until they can get it sorted out, and even then they don't have a set of books for me.

No, work paid for it. But I'm still gonna raise hell. I didn't get into the class until after 3pm today, so missed nearly the whole first day of stuff. That's like 20% of the class, so should get us a 20% refund! Or at least that's the stink I will raise.

I'm fasting right now. Fasting is roughly 4-4:10 AM to 8:45PM. Needless to say by the mid-to-end of a work day I'm really starting to feel the lack of energy. I'm also in my last 3-4 weeks of residency period so almost ready to graduate (and of course, senioritis).

So how the **** do I end up THIS screwed on scheduling? I mean, its nothing new - I've been frequently screwed on scheduling before. My fellow third year gets 3-4, or 5-6 patients. The internets get capped out at 6. Me? I get 10. I get 8. No matter Frequently, I'm straight up doing more patients than all the other students in the office combined. And when I complain about it: "Stop Bitching". "You should be HAPPY you're getting MORE experience".

NO.  That's NOT how it works.

I have a 1pm, 1:10, 1:20, 1:40, 2:00, 2:20, 2:40, 3:00, 3:20, 3:40, and 4:00.  I have become *MAD* skilled at documenting my patient encounters 'on-the-fly'. On optimizing any individual patient encounter to blow through it both thoroughly and efficiently while still making a patient feel heard.

But I have ZERO time to look stuff up and learn.

And then sometimes I fall behind. After all, patients very VERY rarely come in for one thing, and have no extraneous issues or complaints. That, and nursing staff routinely takes 10-30minutes to room a patient which pushes me 1-2 appointment slots back.

 

 

Monday this week I had 1 patient on my schedule in the morning . . . and they gave me 9 add-ons for a total of 10. And three of those visits? Office-procedures (which routinely take 2x time).  And its a testament to my skill that I was still out at 4:45!

Tuesday? 10 patients. My peer? 6.

Today: 11 patients scheduled. Friggin' 11. I'm exhausted just THINKING about it. And its the worst kind of patients too. The kind where the complaint is "Check Blood Pressure" but you probe a little and realize you'll be doing a full Syncope workup and med management and and and. A procedure scheduled right in the middle followed by a patient who last time took LITERALLY ONE HOUR (for his 10 minute appointment~!) because of his nonstop bitching.
I'm feeling drained just looking at this schedule. I feel tired and hungry already. I feel like the stress of these days one after the other without being able to pace myself I might actually need help to power through this.

My peers? 4 patients and 6 on their schedule respectively. Same office. Same Afternoon.

Worst part? No respect. I feel like with this level of schedule disparity the Attendings and office manager should be singing my praises as being the 'best'. That my throughput puts the other residents to shame. That everyone needs to "get on my level". SOMETHING.

Instead? I get told off if I ever mention the unfairness of this all. And if I keep my beak shut and work hard, I get told off on the occasions where I can't keep up because the schedule is literally too much.

In all seriousness - it really is OK. I just have to embrace the fact that when your appointment at the doctor's office is for 3pm, you can't be certain to be seen till 4pm. Once I stop feeling bad about making my patients wait an extra hour because I can't be in two places at once and am not willing to give my patients short-shrift in their care, then I'll feel better.

That, or once I start getting paid for seeing more patients rather than just watching my coworkers 'coast' off of my blood and sweat.
Or I get out of here to somewhere where we at least TRY to split patients evenly. Or don't routinely give Ahmad the most.
